If you are a SGT or above with NCO-ERs on file, you can either phone your request, fax your request, or mail your request. 

If you are a SGT and above with no NCO-ERs on file, or if you are a SPC or below, you must send either a written request by mail or fax. Include your complete name, social security number and your signature in your request.

All three methods will take approximately 4-6 weeks to process. 

Your OMPF will be mailed to your current unit address on file. 

If you do not receive your OMPF within six weeks, please contact your PAC or PSB to find out if your current unit address is up to date.

NOTE: Three to five days advanced notice is required. 

Soldiers not making appointments in advance may experience delays, or may not be able to review their OMPF due to normal servicing or processing. In those instances where a soldier comes to USAEREC without making an appointment and the OMPF is not readily available, the soldier may leave an address so the OMPF can be mailed to their home station.
